Welcome to New Hope for Eastern Oregon Animals' Powder Pals adoption program. We request the folloiwng information so that we can assist you with the selection of a new dog. This form, along with a consulation with a New Hope representative, are designed to help you find the dog most compatible with your lifestyle.

To be considered as an adopter, you must:

1. Be 21 years of age or older

2. Have identification showing your current address

3. Have the knowledge and consent of you landlord

4. Be able and willing to spend the time and money necessary to provide training, medical treatment and proper care of the dog

Completion of this application does not guarantee adoption of a New Hope Powder Pals dog.

Please note that as of 4/1/2013, the adoption fee increased to $250 adoption for Powder Pals dogs. Dogs are trained in basic obedience commands, spayed/neutered, vaccinated, micro-chipped for permanent identification in the event that the dog is lost or stolen. New Hope requests that the adopting family also register and pay microchip registration fee (currently $10.99), as this allows us to earn free micro-chips for our program.
